Hello and welcome to my series of lessons on “Medical Marijuana,” brought to you by www.indicainstitute.com. I am your Prof, a Canadian government certified teacher by trade. I’ve designed this course for those people that would like help and instruction on how to register for medical marijuana, buy it from a licensed producer, grow it yourself, defer growth to someone else, or simply require instruction on how to complete the registration forms. I hope this series of lessons answers the questions you have, and that you enjoy, learn and share my videos.

Hi and welcome to my ‘Medical Marijuana’ series of lessons. The objective of this lesson is to help you complete the application to grow your own marijuana. If you have not already done so, go to my website and download the application to produce medical marijuana for your own purposes. Once you have done this, proceed with the lesson. If you want direction on a specific section, see the timeline listing in the video description and skip ahead to that section.

The first section of the application is called ‘Application Type.’ If it’s your first time applying for this registration, check the box entitled “new,” and disregard the rest of section 1 and move on to section 2. If you’re renewing your registration, check the box entitled “renewal” and print your registration number in the appropriate box. Disregard the rest of section 1 and move on to section 2.

If you are completing the form to amend the original registration information, click the box entitled “amendment.” Print your registration number in the appropriate box. Below this, provide a brief description of what you need to change. For instance “changing my last name” or “changing production site” In the reasons for proposed change box print a brief description of why you are changing the information. For instance “I’ve bought a new house” Write the date the change has, or will take effect in the appropriate box.

Then move on to section to and complete it in its entirety. In the case of amendments only, you are to complete section 1, 2 and the section of the proposed change only. For instance if you are only changing the production site, you will only need to complete sections 1 and 2, and annex A. If you are changing any personal information you will be required to send proof of the change along with the application. This could be a photocopy of government issued ID or certificate of a name change.

For section 2 you will need  to provide your personal information. This section is pretty self explanatory, but I will run through it quickly for those that need the guidance. Write your first, last and middle name. Indicate your gender, or check the box entitled “x” if you do not associate with any specific specific gender. Write your Date of birth and year-month-day. Print your and phone number and email address. Check either English or French depending on which language you prefer to converse in. Enter your ordinary residence information and check the box that best describes your domicile in the options provided below. For a mailing address that is different than where you reside, complete this information as well. If you are amending your registration information in a way that affects your daily specified amount, you need to include the medical documentation with your application. Check the box that indicates you have included medical documentation. Once you have completed this, move on to section 3.

Section 3 is optional and only applies if there is an individual responsible for the applicant in question. If this is the case, they are to complete section 3A and 3B. If this does not apply to you, move on to section 4. In section 3A the responsible individual will provide their personal information in the same fashion stated in section 2. For section 3B, the responsible individual must sign and date in the space provided to confirm they have read the declaration. Then Move on to section 4.

In section 4 you are to check the box that applies to you. If you are producing marijuana for your own needs check the first box. If you plan to have someone else produce the marijuana for you, check the second box. The next line asks if you will need starting materials, meaning plants or seeds, from a licensed producer. Check the box that applies to you. After this it asks if you will need an interim supply, meaning will you need fresh or dried marijuana from a licensed producer while you wait for your plants to mature? Check the box that applies to you. Once you have completed this section, move on to Section 5.

Section 5 states that in the event the police request information related to you, Health Canada will release a limited amount of information. This is put in place so that when you sign you know that Health Canada and the police department may communicate if the police request to do so. It is presumably so that the police will not come knocking at your door when they see more than 4 marijuana plants in your household.

For section 6 check the box stating that all the information you put in the form is correct. Then sign, print your name and enter the date to confirm you understand and have been made aware of this information.

For Annex A, this section is only applicable if you checked off in section 4 that you plan to produce marijuana for your own purposes. If you are designating someone to grow on your behalf, disregard this section and move on to Annex B.

In A1 select one of the options in this section. If you are producing marijuana at your ordinary place of residence, the address you provided in section 2, check this box and move on directly to Section A3. If you plan to produce marijuana anywhere other than your place of residence, check this box and provide the address in the space provided below. Indicate whether you are owner or part owner of this property. If you selected ‘yes’ to ownership of the property, move directly to section A3. If not, move on to section A2.

In section A2 the personal information of the property owner must be provided. If you own the property, make sure you checked this box in the previous section and move on to section A3. Although if you do not own the property, for this section you can fill out the information for the site owner yourself, though the owner must date and sign the section themselves. If the property is co-owned, the co-owner must also provide their information, read the declaration, date and sign.

Section A3 is mandatory to complete. For this section, choose whether you will grow your plants solely indoors, solely outdoors, or a mixture of both indoors and outdoors. You must note that you will not be able to grow outdoors if you live near a school, park, or any other similar location where minors normally gather. Know this ahead of time so you can plan your crop accordingly, and won’t get rejected by Health Canada.

Section A4 is also mandatory. You must state whether you will store your harvest at your place of residence or the production site. Choose whichever option applies to you.

In section a A5, this section is a declaration on your behalf. You must read and check off all of the boxes, sign, print your name and date it in order to send the application off complete. If any of the boxes are not checked off, the application will be deemed incomplete and will not be processed. Once you have completed this section, your application is complete and you can send it to Health Canada at this address.

Annex B  is optional and only applies to those applicants that plan to defer their marijuana production to another individual. If this applies to you, the person you are designating as your producer must complete this section.

In section B1 the designated person must provide their personal information, place of residence and mailing address if it is different from their place of residence.

Section B2 is meant to inform the designated grower that they must provide a police check stating that they have never been convicted of a drug offense or served a sentence for a drug offence. They must submit this check with the application, and check the box in this section that states it is provided in the application.

In section B3 the designated producer must state whether they will grow at their ordinary place of residence, the applicants ordinary place of residence, or another location. If you choose another location, provide the address information in the space provided below before moving on to section B4. If you chose either of the first two options, you can skip section B4 and move on directly to section B5.

In section B4, the personal information of the property owner must be provided if you or your designated producer do not own the property yourselves. If either of you own the property, make sure you checked this box in the previous section and move on to section B5. You can fill out the information in this section for the site owner yourself, though the owner must date and sign the section. If the property is co-owned, the co-owner must also provide their information, read the declaration, date and sign the page.

Section B5 is mandatory, and therefore must be completed. For this section, the designated grower must choose whether they will grow their plants solely indoors, solely outdoors, or a mixture of both indoors and outdoors. Note that you will not be able to grow outdoors if you live near a school, park, or any other similar location where minors normally gather. Know this ahead of time so you can plan your crop accordingly and your application to grow won’t be rejected by Health Canada.

Once this section has been completed the application is complete and can be sent to Health Canada at this address.
